Athletic/Activity Clearance Packets
are required to be completed for ALL groups as they are considered an Activity.

The current Athletic/Activity Clearance procedure can be found at

 (The packet is usually updated around August for the new school year.)

A separate clearance must be completed for EACH Sport or Activity your child participates in. (So if your child is in multiple music groups and or other extracurricular activities listed in the packet found at the link above be sure to check off on the confirmation for each group or activity. And write separate Participation Donation checks for each.)

Marching Band and Color Guard also need physical exam.


THIS IS NOT THE SAME AS THE MUSIC BOOSTER FORMS & DONATION THAT
YOU MAY HAVE TURNED IN AT REGISTRATION or AT BAND CAMP


*NOTE: the procedure may change to be more on-line. Please follow the instructions at the link above from the School website. The Activity Clearance is a school thing not a Music Booster thing.
Attach required forms (be sure that parent/guardian and student sign each form as needed) to a check as follows:

Orchestra


Jazz Band


payable to "PVPHS - Marching/Concert Band" for $200*

payable to "PVPHS - Choir" for $100*

payable to PVPHS - Orchestra" for $ 75*

payable to "PVPHS - Jazz" for  $150*


* The amount is determined by the school and can be found listed on the Athletic/Activity Clearance Procedure Page. Please make sure you use the packet for the current school year. It is usually posted around August.
This is your "Participation Donation":

This amount covers TRANSPORTATION COSTS (festivals, football games) only and are subject to some change each year. They are separate from music booster membership. ​
​
Turn in forms and checks to the STUDENT STORE. (Marching Band & Color Guard need to take their physical form to the Health Office and have the "Confirmation Page" signed off prior to taking it to the Student Store. If you have already submitted a physical form, you still need to have the Health Office sign off on the "Confirmation" prior to going to the Student Store.)

Donations are voluntary and students who choose not to make a participation donation or who make a donation in an amount different from that listed will NOT be denied the right to participate or otherwise be penalized in any way. However, successful programs rely on these donations being completed.
​Donations are tax-deductible; consult your tax adviser for full details. The Participation Donation is made to the school.
